Electronic Application

You may now submit your UM application electronically; however, you must still mail in a Certification Form and the $50 application fee. This easy and efficient option allows you to work on your application until you are ready to submit it electronically. Your data is safely stored on our secure database, and can be referred to at your convenience. Click here for this option.
