All students who will be not be present in class for any reason should complete this online form. This form is submitted online to the Dean of Students Office. The form will be forwarded to the individual faculty members whom you list below to notify them of the reason for your absence. Some faculty rely on this form for the transmittal of information regarding the reason for your absence. Faculty members have different policies with regard to absences. If you are in doubt about the policy in a given class, you should check with the instructor. The Dean of Students does not have the authority to excuse any absences.

If you will be absent from class for more than three days due to a medical or personal emergency, it is very important that you contact the Dean of Students Office directly at (305) 284-4551 to discuss the circumstances.
